Textbook

Note: The textbook
fee is for book rental only.
If any book is lost, stolen, or damaged,
the cost of the book will be assessed.
*A high school student activity pass ($25.00)
is optional and due at enrollment. The amount may be included with the
check for textbook fees. Other student expenditures,
due after enrollment, would include towel fees, club fees, graduation
fees, and project fees for woodworking, metals, vocational agriculture,
and some art projects.
The check for school
lunches should be separate from the textbook fee.

High School Activity Passes
GBHS activity passes will be sold at enrollment only. They are $25 and will admit students to all regular season home athletic events. The activity pass may be paid for with the school enrollment fees.

Early Childhood: 8:05 a.m. - 11:00 a.m. / 12:05 p.m. - 3:00 p.m.
Elementary: 8:00 a.m. - 3:03 p.m.
Middle School: 8:05 a.m. - 3:03 p.m.
High School: 8:05 a.m. - 2:58 p.m
Immunizations
Kansas State Law and School Board policy requires all students to have their immunizations
up to date before enrollment. New immunizations requirements for kindergarten students are as follows:
Five (5) doses of *DPaT
Four (4) doses of polio IPV/OPV
Two (2) doses
of **MMR
Three (3) doses of Hepatitis B
Two (2) doses of Varicella (chicken pox)
*DPaT = diptheria, pertussis (whooping cough), and tetanus.
**MMR = mumps, rubella (German measles), and measles.
Students who were notified of their need for additional immunizations in the spring of 2012 must present proof of immunization before entering school, preferably at enrollment.
Immunizations are available from the Barton County Health Department, 1300 Kansas, Great Bend.
Free Athletic Physicals
2012 - 2013
Free physical examinations for USD 428 student athletes will be held on Saturday, August 4, 2012, from 7:00 to 10:00 a.m.
The physicals are available to middle school and high school students planning to participate in any sport (interscholastic or instructional) during the 2012-2013 school year. Physicals will NOT be given prior to each sport season.Questions regarding athletic physicals should be directed to the Activities Office at 793-1560. All MS & HS physicals will be completed at the HS commons area.

Schedule for Physicals
Grades 10 - 12, 7:00 to 8:30 a.m.
Grades 7 - 9, 8:30 to 10:00 a.m.
Packets for physicals will be mailed the last week of July. All forms should be completed, signed, and returned with the student on Aug. 4. A copy of the completed physical will be provided for the parent.
